At Total Health Choice, our OTs work across Melbourne supporting older adults, people living with disability, NDIS participants, and children aged 3 to 18. We are a mobile service , we come to you, whether that is your home, a residential aged care facility, supported accommodation, or your local community. Our fully qualified Occupational Therapists , all holding a minimum of a Bachelor of Occupational Therapy , provide a comprehensive range of services across aged care, disability, and paediatric settings.
We assess the home environment and recommend modifications to improve safety, accessibility, and independence. This includes grab rails, ramps, bathroom upgrades, and structural alterations , supporting NDIS participants, Home Care Package clients, and private clients.
We assess individual needs and prescribe the right assistive technology , from walking frames and wheelchairs to recliner chairs, mobility scooters, and specialist seating. We manage the trial process and liaise with equipment suppliers on your behalf.
A Functional Capacity Assessment (FCA) provides a clinical picture of a person’s ability to manage daily tasks and activities. FCAs are used to support NDIS plan reviews, aged care funding decisions, and discharge planning.
We support individuals to build, maintain, or regain skills for everyday tasks , including personal care, meal preparation, household management, and community access. The goal is always greater independence and quality of life.
The bathroom is one of the highest fall-risk environments in any home. Our OTs specialise in bathroom safety assessments and deliver detailed reports to support modifications funded through NDIS, Support at Home packages, or private arrangements.
For clients experiencing sensory processing challenges, our OTs conduct sensory profiling assessments that identify triggers, inform care strategies, and support NDIS goal planning.
For kids aged 3 to 18 who prefer to visit a clinic over house visits, our THC Caulfield Clinic is a safe space for your child to build confidence with our experienced Paediatric OTs.

We support older adults through Support at Home Packages and residential aged care, NDIS participants of all ages, private clients, and children aged 3 to 18 through our paediatric OT service. If you are unsure whether our services are right for you, contact our team and we will guide you.
Yes. We actively partner with Support at Home providers, case managers and aged care coordinators across Melbourne. You can submit referrals through our online referrals page or call our team directly. We respond within 2 hours and provide regular progress updates throughout the engagement.
We work with clients funded through Support at Home, NDIS plans, private payment, and TAC. If you have a specific funding type and are unsure whether we can assist, our team is happy to discuss your situation and advise accordingly.
We are mobile across Melbourne, including the south-east, bayside, eastern, northern, and western suburbs. Our OTs visit clients at home, in residential aged care facilities, and at supported accommodation. We also have a paediatric OT clinic in Caulfield.
Our OTs complete a thorough assessment and provide a written report detailing the recommended modifications, including clinical justification for funding applications. Modifications can be funded through NDIS plans, Support at Home, or private arrangements. We work with the client, their coordinator or case manager, and any necessary tradespeople to support the end-to-end process.
All of our OTs hold a minimum Bachelor of Occupational Therapy and are registered with the Australian Health Practitioner Regulation Agency (AHPRA). Several of our senior OTs and team leaders hold postgraduate qualifications and specialist experience in aged care, NDIS, housing assessments, and paediatric OT.
Progress is measured against the goals established at the initial assessment. We provide written reports to clients and their case managers or support coordinators. Goals are reviewed regularly and care plans are adjusted as the client’s needs or circumstances evolve.
