How We Integrate with Support Organisations

Our role is to provide Physiotherapy and Occupational Therapy with Trust, Compassion & Care.

There are many operational processes that we comply with. The primary ways we support coordinators and their participants include:

  • Maintaining our registration as an NDIS registered allied health provider.
  • Attending phone or in-person meetings with other healthcare providers where multidisciplinary care is required.
  • Completing NDIS request forms, functional assessments and recommendations based on the participant’s goals.
  • Liaising with the NDIA where required.
  • Preparing detailed reports with relevant recommendations that address participant needs.
  • Providing progress notes, participant status updates and clinical documentation.
  • Adapting to relevant scheduling software or systems where needed.

The overarching objective of integrating with coordinators is to improve the quality of supports and services provided to the participant.

Coordination of Referrals

We aim to make the referral process clear, responsive and easy to manage for the participant, coordinator and wider support network.

Identifying if We Are an Appropriate Provider

Total Health Choice can manage single referrals as well as larger brokered contracts.

Before proceeding, we first identify whether we are an appropriate provider and can make a meaningful contribution to the participant’s life.

We are happy to meet support coordinators and participants in the way that suits them, often by phone, while encouraging in-person meetings where appropriate.

We can provide information packs about our services. We have primarily worked with adults living with disability and the aged care sector since 2011.

Submitting a Referral

Please submit your referral online or contact us with any questions.

We will email a pre-home-visit risk assessment screen to help support the safety of both our clinicians and the participant.

We schedule appointments as soon as possible, usually within one working week.

After receiving a referral, we may phone to confirm whether additional assessments are required, such as Functional Capacity Assessments.

Services

After attending the property, we formulate a healthcare plan that all relevant parties consent to.

  • NDIS participants use their NDIS plans.
  • Home Care Package clients use Total Health Choice documentation.
  • The participant or participant advocate receives a Service Agreement to sign.

The Service Agreement covers the schedule of supports, responsibilities, policies and procedures, cost of service, and reviewing goals and milestones.

Our involvement may range from one Functional Capacity Assessment through to ongoing assessments and treatment.

Home modification projects may involve additional communication, as we are often the project manager and a central point of contact for coordinators and building contractors.

Ongoing Services

One of our core values is Trust. We aim to earn the trust of Support Coordinators and value long-standing working relationships.

We support coordinators over the long term through:

  • Reassessments to justify funding for assistive equipment.
  • Reassessments to justify funding for Physiotherapy or Occupational Therapy.
  • Helping participants transition to new properties.
  • Supporting changes to mobility and functional status.
  • Providing holistic allied health support.

We appreciate feedback from everyone involved and are privileged to accept referrals from the coordinators we partner with.

Clinical Software / Medical Records

We partner with multiple healthcare organisations.

We are a systemised business and manage scheduling, billing, work health and safety, and internal documentation.

Our systems include:

  • Cliniko for participant information, referral scheduling and progress notes.
  • Google Workspace for document management.

Over time, our systems capture a medical history for the participant. We can share relevant clinical data with the consent of the appropriate parties.
