Starting with Total Health Choice

Total Health Choice works with adults and older adults throughout Melbourne. We were established in 2012.

Our foundations began in Aged Care, but as registered providers of the NDIS we feel that aged care and the NDIS are synonymous with our core values of trust, compassion & care.

We are university-trained, kind and caring, and committed to communicating clearly with the individuals and organisations we partner with.

From first enquiry to ongoing support

Our NDIS process is designed to make next steps clear for participants, families, coordinators and care organisations.

Step 1
Enquiry

If the participant is self-funded, they can contact us directly via email, phone or through our website. If the participant is agency-funded, their support coordinator can contact us directly.

We then organise a suitable day and time to complete a detailed initial assessment.

Step 2
Initial Assessment

At the initial assessment, we set goals and provide a participant handbook outlining our policies, procedures, values, patient rights and how we work together.

We submit required documentation and prepare a formal NDIS service agreement.

Step 3
NDIS Plan & Service Scope

We work on the areas highlighted in the initial assessment. This may involve regular one-to-one Physiotherapy sessions, Occupational Therapy equipment trials or adaptive processes.

We also provide exercise handouts for participants to complete independently or with a carer, friend or family member.

Step 4
Exiting Services / Ongoing Support

In the event the participant needs to move properties or conclude services with us, we help with the transition in either circumstance.

NDIS FAQ

Answers to common questions about NDIS support with Total Health Choice.

In relation to the NDIS, Total Health Choice works with people aged 18 years and over with a disability, particularly middle-aged adults and older adults.

We typically complete a reassessment of progress in a month, and then again in three months. We make sure we are on track to achieve the participant’s desired goals and outcomes.

This milestone tracking is stipulated in the NDIS plan, which is updated regularly.

Total Health Choice is goal specific, with goal-oriented assessments and achievements.

We are guided by the price recommendations of the NDIS and currently charge at the maximum rate.

Participants and coordinators can enquire with multiple NDIS providers to compare price variations and select a healthcare provider that is economically viable.

We can make long-term costs and funding clearer over the phone or during an initial assessment.

These services primarily take place at the participant’s residence to address home independence. When goals relate to participating in activities outside the residence, we may support the participant and complete functional assessments in the community.
