Residential Aged Care FAQ

We really value structured feedback.

Where feedback relates to a member of our team or the service we are providing, we investigate the cause and work to make amends, change and improve.

We believe the key to avoiding issues is to work closely with facilities, managers and care coordinators so we can provide high-quality service. We are hands-on and present.

Total Health Choice’s team leaders use a structured handover process.

This ensures clinicians touch base with appropriate facility staff to update them on assessments, changes in mobility and the work that has been performed.

The handover process helps keep all parties synchronised.

We maintain a transparent review schedule to evaluate the outcomes of interventions.

We conduct a minimum of biannual reviews for facility residents.

For residents in our pain management program, known as the Wellness Centre Program, we complete three-monthly assessments for monitoring purposes.

While each resident has a six-monthly review, we also complete reviews when there are changes in mobility, including:

  • Recent hospital admission
  • Recent fall
  • Problems with mobility equipment
  • Rapid mobility decline due to age or comorbidities
  • Medication changes

Total Health Choice prides itself on the stability and quality of care delivered to the facilities and residents we serve.

Total Health Choice stores its own data using cloud-based software.

We also work with facilities to maintain accurate document management systems. Facility staff can access our notes and assessments through their own system at any time.

We use a priority system that ensures residents are treated in the appropriate order. For example, post-fall reviews or post-hospital assessments may take precedence.

Total Health Choice liaises with families, healthcare companies and other parties on a daily basis.

We are a multidisciplinary team, and we know we are only as good as the team around us.

Our hands-on approach allows us to work closely with other teams. We particularly aim to support nurses, clinical coordinators and unit managers in the facilities we partner with.

Connecting with residents, care staff, the lifestyle team and volunteers is critical to fostering a culture that supports emotional and physical wellbeing.

We know close working relationships are central to delivering the best outcomes for residents.

In 2018, we met a resident who had experienced a lengthy hospital stay.

When she returned to the residential aged care facility, she was struggling to walk and was mostly bound to a wheelchair. She could barely walk five metres unassisted and required two people to assist her with walking.

One of our therapists worked with her and initially prescribed a gutter frame to begin walking rehabilitation.

The resident also participated in our Wellness Centre Program, completing regular exercise sessions and therapeutic massage with our Physio and OT team. We periodically reassessed her to monitor progress towards her desired outcomes.

She achieved her goals and is now walking 500 metres numerous times a day.
