Short-Term Restorative Care (STRC)
At Total Health Choice, we are dedicated to providing comprehensive and individualised care that prioritises your unique preferences and needs.
Our Short-Term Restorative Care (STRC) program embodies our values of trust, compassion, and care, offering you a pathway to improved wellbeing and increased independence.
Total Health Choice offers STRC services such as Physiotherapy and Occupational Therapy.
We were founded in 2011 and have a long-history providing services to the aged-care community and individuals post surgery so we are well-placed to offer STRC services.
Understanding STRC
STRC, which stands for Short-Term Restorative Care, is a government-funded initiative in Australia aimed at delivering early intervention care services.
The primary goal of STRC is to assist older adults for a duration of up to 8 weeks, aiming to help them postpone or circumvent the need for long-term care. Specifically:
- how services will be delivered
- who will deliver the services
- how the services will address the client’s functional decline.
The official government resource can be found here: https://www.health.gov.au/our-work/short-term-restorative-care-strc-programme
This program provides a package of services, including physiotherapy, occupational therapy, and other allied health services, all designed to meet people’s unique needs.
Who is STRC Suitable For?
STRC is suitable for older individuals who are experiencing a decline in their physical condition and aspire to improve their functioning. To determine eligibility, a face-to-face assessment by an Aged Care Assessment Team (ACAT) is generally required.
You can be referred for STRC by your GP, a hospital, or even self-refer for an assessment. The eligibility criteria are inclusive, ensuring that those recovering from hospital stays and those facing physical and cognitive challenges in the community are covered.
Total Health Choice’s Coverage and Approach
Total Health Choice proudly serves the majority of Melbourne through our mobile service, as STRC services take place in people’s homes.
We have a range of Allied Health services, namely Occupational Therapy and Physiotherapy.
Our approach is client-centred and proactive, focusing on health management among older adults living at home. We understand that each individual is unique, and our services are designed to adapt to your preferences and needs.
When is the Right Time to Contact Total Health Choice?
The right time to contact Total Health Choice for STRC services is typically after you’ve undergone an ACAT assessment and received approval for the program.
We encourage those considering STRC but have not yet started the process to reach out to us for guidance about our services offered. We are here to support you at every step.
Our friendly team can be contacted via phone or form on our contact page.
Program Duration and Transition Options
The STRC program typically lasts up to eight weeks, with the possibility of a limited extension of up to four weeks in exceptional circumstances. At the conclusion of the program, we collaborate with you to review your goals and plan for the next steps. This may involve:
Returning to Previous Living Arrangements
If suitable, you may transition back to your previous living arrangements with the support and strategies learned during the program. Our Physiotherapists and Occupational Therapists would ensure your previous living arrangements (i.e. level of support, equipment) is appropriate
Exploring Additional Care Services
We can assist you in understanding other funding schemes such as Homecare Packages. Total Health Choice is able to continue our sessions with you under different schemes.
Referrals to Community or Health Services
If identified, we can help you identify other supports that you may require, such as Orthotics, Speech Pathology and Podiatry.
At Total Health Choice, we care about your long-term success, so we will take a holistic perspective and ensure your long-term healthcare needs are met beyond the STRC period.
Costs & Coordinating Care with Other Providers
The STRC program is government-funded, significantly reducing the costs for eligible individuals.
Total Health Choice is committed to transparency, and we will communicate the fee structure, which is mandated by the government subsidy.
We welcome phone calls to discuss this with our team before making any decisions. We advocate for choice and control!
As part of a STRC program, a care plan is to be authorised and finalised across the multidisciplinary care team, Total Health Choice is experienced in such settings. We actively involve you and your family in the care process, valuing your input and insights to achieve the best outcomes.
