She has started having a wash at the sink instead of a shower.

She will not say why. She will say it is easier, or that she had one yesterday. What has usually happened is that she got stuck in the shower once, or slipped and caught herself on the screen, and decided on the spot that the shower is now off limits.

More older Australians are injured in the bathroom than anywhere else in the house. And the bathroom is the room families look at last, because you do not go in there with your mother.

What we do and where

Total Health Choice provides physiotherapy, occupational therapy, exercise physiology and speech pathology in people’s own homes in Hampton 3188 and Hampton East 3188. Podiatry and massage therapy are both coming. What actually happens in a bathroom assessment

The occupational therapist.

The visit runs about an hour, and yes, we go into the bathroom, with her, with the door open, and we watch her do it rather than asking her to describe it.

We watch her get in and out of the shower. Whether the shower is over a bath, which changes everything. What she holds onto, which is very often the towel rail or the shower screen, and neither of those is built to hold a person.

We watch her get up off the toilet. We measure its height, because forty centimetres against a pair of arthritic knees is a genuinely hard lift and a raised seat or an over toilet frame solves it in a week.

We look at the floor when it is wet, the mat, the lip of the shower base, and whether she has to step over anything.

We look at the light, and whether the switch is outside the door where she has to walk in the dark to reach it.

Then we go and look at the bed, because the bathroom problem is usually a strength problem wearing a costume. If she cannot get off a low bed she cannot get off a low toilet either.

And we walk the path from her bed to the toilet the way she walks it at two in the morning, and we count the obstacles.

The physiotherapist.

We measure the things a physiotherapist measures, because it is the same problem. Timed Up and Go. Thirty second sit to stand. Four stage balance test. Walking speed over four metres.

What we do about it

A written recommendation with the actual items on it. Grab rail here, at this height, on this wall. Shower stool of this height. Raised toilet seat or over toilet frame. Non slip mat. A light with a switch she can reach before she is in the room.

We tell you what each item costs before anything is ordered, and we tell you which ones her Support at Home package will cover and which ones it will not.

A physiotherapy program alongside it, built for what she can tolerate, seated or standing, near a benchtop or a chair for support, ideally five to ten minutes a few times a week, printed with photographs, and built around the specific movements that are failing. Standing up. Stepping over. Turning around in a small space.

The same therapist each visit.

And a monthly progress report to the family with the outcome measures in it, so you can see whether she is getting stronger or whether the equipment is doing all the work. Both are worth knowing. The report goes to you, to her care team, and to her Support at Home Provider.

Yes, and it is worth finding out why rather than pushing. It is almost never about cleanliness. It is usually fear after a slip, or that she cannot get up off the shower seat, or that she cannot manage the step. All three are fixable, and usually in one visit plus a rail.

The occupational therapist assesses, measures and writes the recommendation, including the exact height and wall position. The installation is done by a builder. We can tell you what to ask them for so it gets done right the first time.

It is the hardest bathroom set up for an older adult, and it is very common in Hampton’s older houses. There are usually two options, a bath board with a rail as the quick fix, or a proper conversion as the long one. The OT will tell you honestly which one your mother’s house and budget suit.

Sometimes, and we would rather try. If she can get her sit to stand up and her balance back, she may not need half the equipment. We will tell you which parts are a strength problem and which parts are a house problem, because they are different.
