Home Visit Initial Assessment

One of our wonderful Physiotherapists will visit the residence. The first session is typically a mobility assessment based on the nature of their challenges or complaints.

We specifically help with:

What is Included in Home Visit Physiotherapy?

We perform outcome measures which are our baseline measures allowing us to assess and reassess the progress of an individual’s therapy.  This provides a baseline to set goals to achieve.

We then put a plan in place, which will entail

  • Goals the individual wishes to achieve
  • Frequency of sessions
  • Expected duration
  • When to reassess outcome measures
  • Implement a rehabilitation program between therapist and patient
  • Provide a individualise home exercise program to do with their carer, family or individually

You will then be supported by our Australian team for the duration of your Physiotherapy with us.

Types of Physiotherapy

We assess the patient’s physical attributes (namely upper/lower limb function, balance, transfers, functional capacity and safety). We take outcome measures that allow us to measure our performance over time. From there, we’ll formulate short-term and long-term goals and create a treatment plan. This may include:

  • Strength exercises
  • Respiratory conditioning
  • Balance exercises
  • Joint mobilisation
  • Hands-on therapy, soft tissue massage, manual therapy

Once a plan is in place, together with trust, we will perform the treatments together. For example, for a patient recovering from a stroke, we may work on gait retraining, contracture management and safe transfer practices.

Megan Physitherapy Clinical Manager Megan has spent over ten years providing physiotherapy services to clients in aged care and the community. She is passionate about helping clients reach their goals. Megan is empathetic, caring, and strives to provide personalized care to all of her clients. Megan has completed further study in a number of areas including hydrotherapy, Parkinson’s disease, dementia, and gerontology. She loves learning and putting into practice the new skills that she is taught.
Vatche – Physiotherapy Clinical Supervisor
Vatche Physiotherapy Clinical Supervisor Vatche graduated from Melbourne University in 2015 and began his career working in aged care and private practice, treating a varied musculoskeletal caseload and instructing clinical and fitness Pilates classes. He enjoys working in a team setting, where he is able to share his knowledge and experience as well as collaborate within the multi-disciplinaty team, working with all stakeholders to achieve the best possible outcomes for his patients. Vatche really enjoys getting to know his patients on a personal level and building strong connections with them. He believes that this is a key part of working with them to achieve their health and wellbeing goals. He also enjoys the challenge of having to adapt his therapy provision to suit the needs of his patients broad backgrounds and often complex medical histories.
Naomi – Senior Physiotherapist
Naomi Senior Physiotherapist Naomi graduated from The University of Melbourne in 2002 with a Bachelor of Physiotherapy (Hons), she now brings over 20 years of Physiotherapy experience to the team. Naomi has an extensive range of clinical and life experiences that allow her to work with clients across a range of ages, abilities, cultural backgrounds and healthcare needs. Naomi’s strength is her ability to connect with people, to listen, to understand and offer support that is tailored to their personal situation.
Ryan - Senior Physiotherapist
Ryan Senior Physiotherapist Originally from Hong Kong, Ryan is fluent in Cantonese and English, and also speaks Mandarin. He is passionate about community-based care and is dedicated to making a meaningful impact by empowering individuals to overcome challenges and regain independence in their daily lives. He has worked across a wide range of clinical settings, including acute hospitals, community health, and aged care. These experiences have shaped his holistic and patient-centered approach, where he focuses on not only physical recovery but also improving overall quality of life.
Milo – Physiotherapist
Milo Physiotherapist Milo completed her Bachelor of Physiotherapy at Charles Sturt University in 2023, where she immersed herself in a diverse range of clinical settings, including neurological and orthopedic rehabilitation, acute post-surgical care, aged care within the hospital setting, and private practice. During her studies, Milo also dedicated her time as an Allied Health Assistant, providing support to NDIS participants, which ignited her passion for community-based care. With a genuine commitment to patient well-being and achieving positive outcomes, Milo takes a holistic approach to therapy, focusing not only on physical rehabilitation but also on enhancing overall quality of life.
Manu – Physiotherapist
Manu Physiotherapist During his University years, Manu had varied placement experience in acute, subacute settings in the hospital and in the community. The placements confirmed his passion for working with elderly people and people with disabilities. It gives him joy and fulfillment when he sees them achieve their personal and community goals and brings a positive outcome in their lives. He always believed that exercise prescription and therapy can bring immense change holistically in an individual’s life at any given stage of a person’s lifespan.
Elana – Physiotherapist
Elana Physiotherapist Elana graduated from a Doctor of Physiotherapy at Monash University in 2024. During her study, she gained experience in a range of clinical settings including acute musculoskeletal and cardiorespiratory physiotherapy and subacute neurological rehabilitation. Elana has also worked for several years as an Allied Health Assistant in aged care, and thoroughly enjoyed watching the residents improve in their mobility and participation. Her passion for community work has also stemmed from her ability to connect with people and build meaningful relationships. Through her placements, Elana has developed special interest areas in women’s health and vestibular physiotherapy.
Henry – Physiotherapist
Henry Physiotherapist Henry graduated from Monash University in 2023 but prior to commencing his career as a Physiotherapist had worked as a Myotherapist. As part of his physiotherapy training Henry gained experience working in hospital and community settings. Throughout his journey of training and self discovery, Henry has progressively found a deep passion in helping clients in the community as he enjoys taking them on a pathway to help them achieve their goals. Ella Physiotherapist Ella graduated with a Bachelor of Physiotherapy from Otago University in New Zealand in 2022. She began her career as a Rotational Physiotherapist at Nelson Hospital, gaining valuable hands-on experience in various clinical settings, including acute surgical care, neurological rehabilitation, and musculoskeletal outpatient services. Ella recently relocated to Melbourne to broaden her clinical expertise and expand her professional network. Throughout her studies she developed a diverse range of clinical experience across several settings, specifically in both acute and outpatient rehabilitation Hayley has also worked for several years with football clubs as a sports trainer and as an allied health assistant in aged care. Physiotherapy FAQ

  • In what ways can Physiotherapy sessions be unique?

    Occasionally we may add some music as part of our sessions to make it more fun, if you’re lucky, you might see some of our therapists singing and dancing! We may use boxing gloves, play with tennis racquets or use a ball or balloon to train reflexes. If you’re advanced you may need to negotiate one of our specialised obstacle courses!

    We try to cater to each individual to ensure its a session they look forward to.

    For private home visits under the NDIS, there is funding to access amazing assistive technology. We particularly like reaction-based tests that synchronise to mobile phones, so there is quantitative data to record.

  • Will the Physiotherapist use equipment or recommend me to obtain any?

    Yes and yes.

    We often recommend specific exercise equipment to help with your home exercise program. We also recommend equipment including gait aids, wheelchairs, etc.

    In one of our Physiotherapist’s bags, you’ll probably find, balls, weights, therabands, witches hat cones for obstacles and balloons (why we carry balloons is a story for when Total Health Choice visits). We may also bring a foam mat for balance practice or a step for stair practice.

  • What are goals & how are they reviewed?

    Examples of goals

    Goals are patient-directed. For example, a patient who has had a lengthy stay in hospital has resulted in deconditioning. Consequently, they will struggle when they return home as their mobility and strength have declined.

    In this scenario, the goals would revolve around restoring the norm, i.e.

    • Getting up in the middle of the night to go to the bathroom safely
    • Reduce the fear of falling
    • Feeling empowered
    • Feeling safe in the home environment
    • Getting back outside into the community

    Occasionally our patient’s goal may be to keep them living at home as independent as possible by maximising their functional activities within their home environment – improving their activities of daily living.

    We cater to each individual’s needs and provide care to suit the individual.

    How goals are reviewed

    Goals are reviewed by retesting outcome measures. Goals are often performing routine activities in a safe way, so being able to achieve the functional task we’ve set out to do, such as

    • Get out of bed safely
    • Go to the bathroom at nighttime safely
    • Walk 8 minutes to the local cafe
    • Using kitchen equipment safely
