Introduction

The dedicated Occupational Therapy (OT) team from Total Health Choice support individuals through private home visits, NDIS participants, homecare package clients as well as aged care facilities throughout Melbourne.

All of our Occupational Therapists are fully qualified and hold a minimum of Bachelor of OT. Our OTs specifically address:

  • Environmental assessments
  • Functional rehabilitation
  • Equipment requirements (i.e. prescribing appropriate wheelchairs, gait aids etc)
  • Lodging assessment and request forms to the NDIS/NDIA
  • Equipment trials
  • Project management & quotes required for home modifications

The objective of home modifications is to support people in feeling confident, safe and mobile while accessing or living in their home. This ultimately allows individuals to live at home more independently and for longer!

Chaya Senior OT Team Leader Chaya Greenbaum is a Senior OT, specialising in NDIS and aged care assessments, home modifications, assistive technology (AT), and equipment prescription.
Sarah Occupational Therapist Passionate about holistic care, Sarah strives to build meaningful connections with clients, fostering a supportive and empowering environment.
Gabi Occupational Therapist Gabi graduated in 2022 with an Honours degree in Occupational Therapy and has experience across various community settings.
Julia OT Team Leader Julia graduated from La Trobe University in 2016 with a Bachelor of Health Science and Master of Occupational Therapy.
Lee Occupational Therapist Lee graduated from Monash university in 2020 and began her career working in an acute mental health setting.
Ashley Occupational Therapist Experience in community health, NDIS, and subacute hospital settings with paediatric and adult patients. Additionally, he has completed a research project in pressure care education and management.

Occupational Therapy FAQ

  • Do I need a referral from a GP to book Occupational Therapy services?

    No, you don’t need a referral from a GP.

    You can refer yourself, we receive referrals from individuals, NDIS participants, support coordinators, support workers as well as home care package providers.

  • What education and professional memberships do your Occupational Therapists possess?

    All of our OTs  registered with AHPRA and required to undertake mandatory professional development each year to maintain their registration.

    Our OTs are also members of Occupational Therapy Australia.

    All our team possess a criminal record check, working with children check, current first aid as well as personal and public indemnity insurance.

  • I don’t own my property, can I still get Occupational Therapy?

    Most definitely!

    Most of the work our OTs do has low environmental impact and this is a non-issue. In the event we are conducting home modifications, we will liaise with the respective property owner and perform this aspect of the service. We are aware of Victorian legislation that governs this form of healthcare.

  • What is the Occupational Therapy process in detail?

    Step 1 – Enquiry

    Please use our contact or referrals page, or call us directly to start the process. If you are an NDIS participant, please provide your NDIS number.

    We’ll set up the initial assessment within 5 days.

    At our discretion, we may perform a pre-home visit risk assessment over the phone. This may take up to 15 minutes.

    Step 2 – Home Assessment

    One of our compassionate Occupational Therapists will visit the residence to conduct an initial assessment. We will perform a home environment risk assessment – look at the environment to identify hazards and see if its safe for the individual, what assistive equipment could be helpful and if home modifications would be beneficial.

    At this point, we will know what ongoing training will be required. Towards the end of the session, we will formulate a plan of how we will work together, and set meaningful goals. A goal could be improving upper limb dexterity to handle a kettle safely and pour a cup of tea. If the person is an NDIS participant, we will review their current NDIS plan.

    Step 3 – Occupational Therapy Services

    Our OTs mainly help with prescribing, trialling and implementing assistive equipment and technology. Examples of these include introducing ramps, handrails, wheelchairs, cushions for wheelchairs, shower chairs and toilet commodes.

    We are also capable of delivering home modification projects, we liaise with the necessary property management and other third parties. We can create Scope of Works documents and submit these to the NDIA.

    Step 4 – Ongoing Support

    The frequency of rehabilitation sessions with our Physios & OTs varies, this information will be conveyed in our service agreement from the initial assessment. We work well with other healthcare companies and support our patients transitioning from our services.
