Our experienced Occupational Therapists complete a full NDIS and aged care home modifications assessment, provide clear recommendations, and coordinate works with builders, engineers and Support Coordinators. The process is designed to be smooth, supportive and stress-free from start to finish.

Our Home Modification Process

We make every stage simple, collaborative and person-centred.

  • Step 1

Enquiry and Consultation

Contact us directly or through your Support Coordinator or case manager to get started.

Our admin team will schedule an initial assessment. This one to three hour session includes a detailed home assessment, measurements and a discussion about your goals.

  • Step 2

Assessment and Reporting

Your NDIS or aged care Occupational Therapist will complete a thorough home assessment and create a Scope of Works in line with NDIS and aged care home modification standards.

  • Step 3

Approval and Coordination

We submit reports and recommendations to the relevant body, whether that is NDIS, My Aged Care or a residential aged care facility, and manage communication for approvals.

We also coordinate with trusted NDIS and aged care builders and home modification providers in Melbourne to obtain quotes and manage documentation.

  • Step 4

Implementation and Handover

Once the works are approved, we oversee the installation and ensure all modifications meet clinical and safety standards.

Our team can provide manual handling training for carers and staff so everyone feels confident using new supports such as hoists, stair climbers and mobility aids.

  • Step 5

Follow-Up and Reassessment

After installation, we check in with you to make sure your modifications are meeting your needs.

If your mobility or condition changes, we will reassess and update your recommendations.

Areas of the Home We Modify

Minor Home Modifications

Small changes can make a big difference. These NDIS and Support at Home minor home modifications are low-cost, quick to install and help reduce fall risks immediately.

Examples include:

  • Grab rails and handrails
  • Lever handles and non-slip flooring
  • Step wedges or threshold ramps
  • Adjustable or handheld showerheads

These modifications are often funded through an NDIS plan or Support at Home Package.

Major and Complex Home Modifications

For long-term independence and safety, we also deliver NDIS and aged care complex home modifications such as:

  • Ramps, lifts and stair climbers
  • Hoists and ceiling track systems
  • Bathroom and wet-area redesigns
  • Accessible kitchen layouts
  • Widened doorways and home access improvements

We manage the entire process from design to delivery and follow the NDIS home modifications price guide to support transparency. Our team collaborates with builders, engineers and your support network to achieve the best possible outcomes.

Assistive Technology and Environmental Modifications

Sometimes, accessibility can be improved with technology rather than construction.

We can recommend and install:

  • Smart lighting and sensor controls
  • Intercom and communication systems
  • Automated doors and switches
  • Environmental control units and safety alarms

These disability and aged care home modifications can be funded under Assistive Technology in an NDIS plan or through a Support at Home Package, depending on the available funding.

Temporary or Portable Modifications

If you live in rental or temporary housing, our team can assist with portable home modifications that are flexible and easy to remove.

Options include:

  • Portable ramps
  • Shower commodes and over-toilet frames
  • Removable rails and bed poles

Many of these supports have low environmental impact and can move with you to a new home. We can also help arrange hire equipment so your independence travels with you.

  • Support beyond installation

Supporting Adaptation to New Home Environments

When you move into a new home, our Occupational Therapists provide hands-on training to help you and your carers adjust safely.

We make sure everyone feels confident using supports such as ramps, hoists and stair climbers so you can live safely and independently in your new environment.

Frequently Asked Questions

Changes to your home that make it safer, more comfortable or easier to move about are called home modifications. These may reduce falls, provide greater independence and make daily routines easier.

Home modifications are considered reasonable and necessary under the NDIS if they support daily living in your own home.

They are also an important part of supporting safety and accessibility for older people receiving Support at Home services.

NDIS and Support at Home minor home modifications are small, lower-cost changes such as grab rails, handrails, lever taps or threshold ramps. They are generally quick to install and can make an immediate difference to comfort and safety.

NDIS and Support at Home complex home modifications are larger structural projects such as ramps, hoists, accessible bathrooms and widened doorways. These require detailed assessment and reporting by an Occupational Therapist, as well as input from builders.

Yes. NDIS and Support at Home guidelines require a qualified Occupational Therapist to carry out a home modifications assessment before funding is approved.

Your OT ensures the recommendations are safe, practical and designed around your personal goals.

At Total Health Choice, our Occupational Therapists complete the assessment, prepare the required documentation and coordinate with builders and Support Coordinators to make the process simple and stress-free.

Your NDIS and aged care Occupational Therapist is your main point of contact throughout the process. They visit your home, discuss your goals, assess the environment and design practical solutions that suit your daily life.

They also prepare your detailed report for Support at Home and Scope of Works for NDIS review, help organise builder quotes, and check in after work is completed to make sure everything is safe, comfortable and works as intended.

The NDIS uses the term reasonable and necessary to describe supports that genuinely improve independence, safety or quality of life.

Our home modification providers assess your needs to determine what will make the biggest difference to your daily function and long-term wellbeing.

Recommendations must directly relate to your disability and represent good value for money. We work closely with you and your Support Coordinator to make sure every proposal is practical, achievable and focused on your goals.

For Support at Home, approval depends on assessed needs and the available funding level under the relevant assistive technology and home modifications funding pathway.

You can request NDIS home modifications through your Support Coordinator, Local Area Coordinator, or by contacting our friendly team directly through our NDIS page.

We guide you through every step, from initial assessment to preparing reports and submitting your application for approval.

Yes. For NDIS complex home modifications, builder quotes or cost estimates are usually required before funding can be approved.

Your Occupational Therapist will help gather accurate quotes that meet relevant requirements, and we will coordinate with experienced builders to keep the process clear, transparent and compliant.

Yes. You can choose your own builder, provided they are qualified and experienced in NDIS home modifications.

Alternatively, we can connect you with safe, reliable and quality home modification providers in Melbourne.

Either way, our team collaborates with your chosen tradespeople to make sure everything runs smoothly from start to finish.

Once modifications are complete, your Occupational Therapist will return to your home to make sure everything is safe, functional and meets your goals.

They will check that any new equipment is set up correctly and ensure you and your carers feel confident using it.

Where extra training or adjustments are needed, we will organise that too. Our goal is for you to feel completely comfortable and supported in your space.

The timeline depends on approval timing, the type of work and the level of complexity.

  • Minor home modifications can often be completed within a few weeks.
  • Complex home modifications may take a few months because they require detailed assessments, builder quotes, approval and construction time.

Our team keeps you informed from start to finish, with clear timelines and regular updates.
